What to Bring to Your Appointment
If you are a first-time patient and have not or could not complete your new patient forms, you will need to arrive 30 minutes in advance of your scheduled appointment time. Please bring the following items to the appointment:
- Picture ID (REQUIRED)
- Insurance card(s) (REQUIRED)
- Any required co-payment (REQUIRED)
- Debit or Credit Card to place on file (REQUIRED)
- List of all prescription and over-the-counter medications that you are currently taking
- Pertinent medical and surgical history information
Insurance and Payment Information
We are a provider for Medicare and most major insurance plans. We encourage all patients to confirm their coverage prior to making an appointment. Co-payments are due at the time of the appointment. You are responsible for any unpaid balance not covered by insurance.
Please call us with any questions regarding which insurance plans we accept or other billing concerns. Insurance coverage and benefit questions should be directed to your health insurance provider.

Podiatry Associates will require all patients to provide an active card on file.
What does this mean for you?
Your credit card information will be securely stored with our processor. Once entered, our staff will not have access to your complete card details. The card will only be used at your request or if there is an unpaid balance on your account.
